Custom lender condo questionnaires, filled from the association's last 1076
Upload the lender's form. Questionnaire Desk answers each question from the association's latest Fannie Mae 1076 and the answers you approved before, and flags what it isn't sure of for your review.
One lender form, from upload to the questions left for you.
Illustrative example data: a fictional association and lender. No sound.
Your 1076 already holds most of the answers. It just isn't in the lender's order.
Each association's profile is its latest completed 1076. Every lender form after that is answered from it and from what you approved before.
Upload each association's 1076
The fillable Fannie Mae 1076 or Freddie Mac 476 is read from its own fields. You check the facts once and confirm.
Upload the lender's form
Any custom questionnaire as a PDF: fillable, flat, or scanned in grayscale or color (up to 12 pages). Word forms: save as PDF first. You see how many questions were read before you approve.
Review what it proposes
Every answer shows its source and a confidence. Approve the high-confidence ones in one click; the rest one by one.
Download and send it yourself
The filled PDF, still editable and unsigned, and a numbered answer sheet in the form's own numbering.
Less copying from the last 1076, and the risky questions left to you.
Fills fillable PDF forms in place; flat and scanned PDFs get a numbered answer sheet to attach
Every answer shows its source: a line on the association's 1076 or an answer you approved before
Litigation, structural, special assessment and other judgment questions are left for you, never guessed
Approved answers are saved per association and per lender, so that lender's next form needs fewer edits
You sign. You send. You answer for the association.
No signatures
Signature fields are left empty on every form, and the answer sheet has a blank signature line.
Nothing sent
Downloads only. Nothing goes to a lender from Questionnaire Desk.
No guessed judgments
For litigation and similar questions it shows what the 1076 said, as a reference, and waits for your wording.
No borrower data kept
Borrower and loan details are typed at download and go only into that file.
See it on a fictional 48-unit association: its 1076 on the official form, a fillable and a flat lender questionnaire, the review screen and every download.

Before you start
Where do the answers come from?
From the most recent completed Fannie Mae 1076 or Freddie Mac 476 you upload for each association, and from answers you approved on earlier forms. Upload a newer 1076 and its figures replace the old ones. There is no connection to your accounting or management software.
Does it sign or send anything?
No. You review every answer, edit what needs editing and approve the form, then download the filled PDF and send it to the lender yourself. Nothing goes to a lender from Questionnaire Desk.
Which forms work?
PDFs. Fillable PDF forms are filled in place. Flat or scanned PDFs come back with a numbered answer sheet that matches the form's question numbers. Save Word forms as PDF first.
What happens to borrower information on the form?
Borrower names, loan numbers and contact details are never saved to the answer library. Uploaded and filled forms are deleted after 30 days.
How many forms does $149 a month cover?
Up to 100 forms a month across all your associations, by card, cancel any time from the billing page.
